Description
The Outreach Worker position is a key role within a multidisciplinary team located in Holyoke, Massachusetts.
As an Outreach Worker, you will be the primary support for individuals with mental health diagnoses who live independently in the community, focusing on their physical and mental well-being, daily activities, and overall independence and recovery.
RESPONSIBILITIES:
REQUIREMENTS:
The ideal candidate should have a college degree in human services or a related field, along with excellent written and verbal communication skills, empathy, and a dedication to helping those in need. Strong teamwork and independent work abilities are essential, as well as proficiency in computer skills. A valid driver's license and access to a vehicle for work are mandatory. All candidates must undergo a background screening process.
