Practice facilitation, also referred to as quality improvement peer coach, is an approach to supporting improvement in primary care practices that focuses on building organizational capacity for continuous improvement. As a practice facilitator, you will establish a long-term relationship with your practices, becoming a resource for ongoing quality improvement (QI) and evidence translation. PCPF's are specially trained individuals who work with primary care practices to make meaningful changes designed to improve patients outcomes. Is accountable for facilitating the implementation of practice transformation including baseline assessment of current environment, and ongoing facilitation of practice improvement methodologies to ensure compliance with regulatory and NCQA standards. Dispute Resolution For day-to-day operations, the Patient Centered Transformation Facilitator (PCPF) will report to the Patient Centered Transformation Practice Lead who reports to the Manager of Outcomes. The PF will support the development and maintenance of the Patient Centered Medical Home accreditation, and when applicable the Patient Centered Specialty Practices. The PF will support all members of the care team and provide educational support for all quality initiatives tied to the practices they support. Practice locations will vary based on an ever-changing mix of practice locations. Completes other special assignments and projects as assigned, including but not limited to work on Commercial and Medicare quality contracts, Roster Management, Health and wellness campaigns.


Responsibilities:

A day in the life of a
Practice Facilitor at Hackensack Meridian _Health_ includes:


  • The Practice Facilitator will:
  • Act as mentor and provide training to assigned practices on the contents of specific transformational and quality improvements projects.
  • Will meet role competency requirements within 90 days of hire (Practice Facilitator Training Competency Checklist).
  • As a peer mentor the Facilitator is required to display evidence of core traits and behaviors consistent with the 5 HMH Cultural Beliefs during the 90day orientation period: Creative, Compassionate, Collaborative, Courageous, and Connected (Practice Facilitator Training Competency Checklist)
  • Speak with confidence and exhibit advanced communication skills while delivering educational content to providers, managers and team members.
  • High degree of emotional intelligence is needed for this role. selfawareness selfmanagement social awareness relationship management.
  • Assist practices in redesigning clinical documentation and or workflow to support desired changes.
  • Mentor staff modify clinical documentation and or process to ensure sustainability of changes.
  • Clinical documentation expertise support process improvement tactics to optimize EMR clinical data capture to support of regulatory quality measures
  • Identify exemplary clinical documentation and processes in your practices and spread them to others.
  • Identify resources practices need to implement improvements
  • Mentor practice teams to integrate process improvement workflows occurring within them into a cohesive whole.
  • PCMH Practice audit quarterly: assure practice is maintaining NCQA standards.
  • Work with team to acquire recognition of excellence in NCQA Diabetes and Heart Stroke
  • Submit quarterly and annual workbooks to NCQA to recredential and maintain recognition
  • Review monthly patient satisfaction results and work with the practice to improve through the development of action plans.
  • Document competency of team members in Regional Action Plans
  • Assist team in gaining CMS EDIM access as needed.
  • Report eCQMs (outcomes measures).
  • Monthly reporting of outcomes measures to assigned practices. Action planning based on measure findings.
  • Report operational quality quarterly.
  • Answer questions about practice's quality improvement efforts.
  • Each quarter, the PF will report information (action planning) on what the practice is doing to improve those eCQMs (outcomes measures).
  • Display traits and behaviors necessary to form and maintain a longterm relationship with your practices.
  • Display traits and behaviors consistent with HMHN 5 Culture: Creative, Compassionate, Collaborative, Courageous, and Connected.

Qualifications:

Education, Knowledge, Skills and Abilities Required:

  • 5 years of healthcare experience
  • LPN or CMA (Certified Medical
