Exciting Job Opportunity:


We're looking for a Service Coordinator to take care of managing supportive services for the residents of our property, especially focusing on those who are frail and at risk.

The responsibilities include arranging personal assistance, homemaking, meals-on-wheels, transportation, health screenings, and advocacy.

This role is for our properties at:

  • 5801 N Pulaski Rd, Bldg M, Chicago, IL 6064 and 5801 N Pulaski Rd, Bldg H, Chicago, IL 60646
Salary $22.25 per hour / $46,280 per year


Key Responsibilities:


  • Establish and maintain relationships with local, state, and federal agencies, as well as service providers like Area Agency on Aging. Find the best deals for supportive services to ensure personalized care.
  • Create a directory of community-based providers for staff and residents to access. This directory will include various services such as case management, personal assistance, meal services, transportation, counseling, nursing, and advocacy.
  • Understand and enforce resident lease and house rules. Report any violations to the Community Manager.
  • Identify residents in need of support services, especially the frail and at risk individuals.
  • Manage services for residents including needs assessment, service arrangement, monitoring service quality, and ongoing support.
  • Keep detailed records of each resident's case management progress and any related issues.
  • Organize wellness screenings and educational sessions for the residents.
  • Help residents establish support networks within the community.
  • Collaborate with the Community Manager to ensure residents receive necessary services.
  • Provide regular reports to the Service Programs Coordinator and Community Manager.
  • Complete any other assigned duties.

Qualifications:


A Bachelor's Degree in social work, gerontology, or related field is preferred. Equivalent work experience of five (5) years may be considered. Must have prior networking skills and experience in arranging services for the elderly.



Supervision:


The Service Coordinator reports to the Community Manager for administrative matters and Regional Service Coordinators for program-related issues. In the absence of the Community Manager, supervision will be provided by the Regional Manager.
