Summary:
This position functions as a collaborative team partner at Foothill Family Shelter (FFS) and includes daily oversight of clients and portions of the housing program.
Reporting directly to the Executive Director, the Case Manager addresses the needs of FFS's clients.In addition to the below duties, the Case Manager demonstrates experience initiating and sustaining a high level of accountability resulting in positive outcomes.

Intakes:
- Support the Help Desk and assist with phone screenings as needed
- Attend and actively participate in intake interviews
Case Management:
- Maintain a case load of up to twenty clients while providing excellent service
- Provide individualized support by helping each client learn how to address their barriers, maintain and sustain permanent housing
- Maintain efficient case notes by monitoring and documenting clients progress in meeting goals
- Assess and assist with savings plans, budgeting, and other financial related goals
- Assist clients with credit issues
- Ensure that all clients are meeting program requirements
- Assist families with resources for childcare and schooling
- Assist clients with opportunities for job and/or educational advancement
- Make recommendations regarding client progress
- Implement highest standard of customer service when interacting with clients
- Coordinates therapy sessions for clients
- Act as an advocate for your clients
- Meet regularly with the Client Service Team to discuss your caseload
- Help coordinate needed maintenance per client needs
- Maintain confidentiality of records
Property Management:
- Assist with monthly cleaning inspections at the property
- Help ensure apartments are movein ready for assigned clients
LEARN:
- Oversee, coordinate, and implement FFS's Life Skills, Education, Resources, Attitude, & Nutrition sessions each month
- Schedule professional speakers well in advance with topics that relate to our clients needs
- Attend and record attendance
- Report back any findings and continuously monitor to improve the program
Outreach Presentations
- Attend and assist as needed
Other
- Provide aftercare services for past residents on an as needed basis, for up to 5 years;
- Attend and participate in all staff meetings
- Assist with holiday activities and yearround family and children's enrichment activities; helping to coordinate and facilitate
- Help develop and sustain systems to ensure deliverables are consistent and effective to our mission; support followthrough and help measure outcomes and successes
- Attend fundraising events as needed
Qualifications:

Education & Experience:
Bachelor's degree in Social Work, Human Services, and/or Sociology preferred.
