    Description

    The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems.

    The Clinic Nurse must possess the ability to perform focused assessments and evaluate the healthcare needs and or issues of persons served.

    The Clinic Nurse must possess the ability to communicate with members of other disciplines to ensure optimal care for persons served.

    The Clinic Nurse must possess the ability to communicate information gathered to home facility physicians and staff.

    The Clinic Nurse must possess knowledge of anatomy, physiology, and pathophysiology, nursing techniques and procedures, medications and side effects, adverse reactions and signs of toxicity associated with medications, and knowledge of or ability to learn the techniques of administering medications to developmentally disabled individuals.

    The Clinic Nurse must possess knowledge of mathematics. The Clinic Nurse must possess the ability to prioritize work and practice time management to ensure timelines are met. Must communicate, verbally and in writing, in a clear, concise manner. The Clinic Nurse will maintain a professional, harmonious working relationship with facility employees as well as outside agency employees.

    Responsible for the delivery of nursing care necessary to meet the health care needs of persons served. Responsible for focused assessment, care intervention, evaluation and documentation of healthcare needs and/or issues of the people served. Responsible for reporting health care needs and/or issues to the appropriate person(s).

    Will notify the Interdisciplinary Team (IDT), works closely with members of other disciplines of the IDT to assure that communication is in place to provide a more consistent, higher quality care for all persons served.

    Works closely with Clinic Clerk to ensure lab and diagnostic test are entered into the database. Responsible for the management of the clinic and works directly with the Primary Care Provider.

    Provides complex vocational nursing work. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.

    Works under the direction and supervision of the Registered Nurse and performs Nursing duties within the assigned areas and assigned shift.

    Performs basic and complex nursing tasks to fulfill the health needs of the individuals, including and health screenings. Keeps informed of and follows all agency, facility and unit policies and procedures. Assists the RN Supervisor with on-the-job Nursing instruction and training of other new LVNs.

    Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the QA Nurse in developing and implementing quality assurance programs, and assists in the implementation of therapeutic nursing programs.

    Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift, and/or overtime in accordance with agency leave policy and performs other duties as assigned.


    Monitors the health status of all persons served and triages for the PCP individuals who need to be seen in the clinic.

    Tracks the completion of paperwork by the PCP to include but not limited to Annual/Quarterly/Admission/Discharge Physicals, Medicare, Drug Regimen Reviews , medication reviews, Seizure reviews and tracks timely review of lab and diagnostic test.


    Performs focused assessments and nursing procedures skillfully and thoroughly in accordance with training and facility guidelines to assure delivery of appropriate and optimal nursing care.

    Demonstrates thorough knowledge of the nursing process and nursing procedures.


    Participates in Quality Assurance, Peer Review and other activities related to the development/enhancement/refinement of nursing policy and procedure in order to evaluate and improve the quality of nursing care provided on campus.

    Establishes and maintains effective work relationships with persons served, co-workers, supervisors, ancillary staff and visitors. Serves as a mentor for other members of the nursing community. Participates in proactive problem solving related to care of persons served and nursing processes and issues.

    Demonstrates awareness of safety and infection control practices to ensure a safe working environment.

    Participates in training and continuing education activities and staff meetings in order to maintain competence in nursing practice and develop a knowledge base of current trends in nursing care.

    Demonstrates compliance with time and attendance policy and assigned work hours in order to provide adequate nursing coverage in the assigned unit.

    Performs technical Nursing procedures according to facility and state regulations after demonstration for competency, i.e., tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments.

    Must be competent in use of all medical equipment, after competency is demonstrated.

    Documents and reports pertinent observations. Is able to recognize symptoms of illness and report accurately to RN and/or Primary Care Provider.

    Documents according to facility policy.

    Collects data for RN/Primary Care Provider.

    The clinic nurse may have to train any floor nurse to back-up in the clinic.)

    Collects specimens for laboratory examination and conducts appropriate screening tests (after competency is documented) when requested by lab personnel.


    Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

    Follows safety and Infection Control Procedures of facility.


    Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

    Such participation may require an alternate shift pattern assignment and/or location. Must have nursing experience. Considerable knowledge of nursing procedures, techniques, principles and processes used in providing individual care. Must possess the knowledge to provide individual care, prepare and maintain records, communicate effectively, and train others. Must be able to demonstrate a working knowledge of emergency procedures and emergency equipment.


    Registration or Licensure Requirements:
    Two (2) years nursing experience required

    Experience in focused assessments and documentation of health status.

    Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS OIG List of Excluded Individuals/Entities.

    Males between the ages of must be registered with the Selective Service.

    All State Supported Living Center employees are subject to Random drug testing.

    The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
