- Implement extensive opportunity analysis to pinpoint areas for enhancement in population health management across various provider groups.
- Generate reports from payor and population health platform to extract valuable insights.
- Deliver practice performance reports, financial summaries, and economic impact assessments regarding performance and contract aspects to relevant practice stakeholders, both clinical and administrative.
- Conclude end-of-year supplemental data transmission to payors.
- Present outcome-driven summaries of performance initiatives to showcase organizational influence.
- Cultivate enduring relationships and interactions with care teams in practices to elevate financial and quality performance across performance metrics.
- Act as a knowledgeable resource and subject matter expert on population health management, disseminating insights and best practices to providers, staff, and other stakeholders.
- Devising and executing strategies, in collaboration with clinical counterparts, to enhance clinical workflows and care coordination processes that support population health endeavors.
- Work alongside interdisciplinary teams to formulate and implement population health schemes and actions aimed at boosting health results and reducing inequalities.
- Oversee and assess the efficacy of population health interventions and initiatives, making necessary modifications to achieve desired outcomes.
- Bachelor's degree in a relevant field required; Master's degree preferred.
- Registered Nurse (RN) highly desirable
- In-depth healthcare knowledge and comprehension of value-based care agreements across all business sectors (commercial, ACA, MA).
- Proficient in analytical skills, capable of deriving insights from extensive datasets and identifying opportunities.
- Strong presentation skills and the ability to communicate effectively at an executive level.
- Exceptional communication and interpersonal abilities, adept at building relationships and gaining trust with provider practices.
- Demonstrated capability to prioritize and handle multiple tasks in a fast-paced setting.
- Proficiency in project management.
- Proven problem-solving skills and adeptness in navigating complex healthcare systems.

- Willingness to travel locally as required and flexible availability to accommodate patient and provider schedules.
- Requires 50% travel within your designated area (State of GEORGIA).
