Nursing Case Manager Health Services DepartmentDuties:
  • Responsible for identification, organizing and coordinating resources and services to meet healthcare needs of high risk students across the district Work in collaboration with students, family/caregiver(s), district staff, clinic/hospital/specialty providers and community resources to create a team approachQualifications, Education and Experience: RN licensure in the Commonwealth of Massachusetts-BSN required DESE licensed as a School Nurse or eligible for licensure Current Healthcare Provider CPR/AED certification required Must have reliable form of transportation One (1) to three (3) years experience in case management, pediatric home care, and/or school nursing required.
Strong demonstrated knowledge of community health services required.

Excellent interpersonal, organizational and communication skills and ability to collaborate effectively Salary:
Per MNA Contract
