    Description
    This position would be supporting the PACE program in West Seattle, WA.


    The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies.

    The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.

    The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes.

    The PT assumes responsibility for participants treated by the PTA and PT aide.

    Required qualifications:
    Bachelor's Degree in Physical Therapy

    Within 30 days of hire:
    National Provider BLS - American Heart Association

    Upon hire:
    Washington Physical Therapist License


    Preferred qualifications:
    Master's Degree Or Physical Therapy

    1 year of working with a geriatric population

    About the Team


    Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.


    HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.

    Our core competencies include restorative care, longitudinal care and care through the end of life.

    Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
