Tribute Home Care is looking for a customer-service focused, dedicated and collaborative
Case Manager to join our Massachusetts team.

This is a great position for someone with strong case management experience and administrative skills that is looking for a non-clinical position with the opportunity to grow professionally.

A day in the life of an Case Manager


In this role you will work with prospective and current clients, their families, and other professionals involved in their care to ensure they have a positive experience from the start of their home care services.


This will be done by:

  • Providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing inperson assessments
  • Overseeing the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans
  • Providing regular, proactive communication to all parties involved in each client's care
  • Revising care plans as often as necessary
  • Developing strong and trusting relationships with Tribute's clients and their families


This role will involve regular travel in the MetroWest Boston, Boston, and North of Boston area to assess and visit clients.


This person has the flexibility to work from home when not visiting or assessing clients, meeting with referral sources, or attending in-person meetings at our Framingham office.


What you need to be qualified for this position

  • Bachelor's degree from an accredited college
  • Experience working in a clinical setting
  • Prior experience in case management, preferably in elder care
  • An eagerness to impact growth
  • Unwavering, passionate commitment to the customer experience and service excellence
  • Strong writing and communication skills
  • Strong attention to detail and organization
  • Collaborative spirit
  • The ability to travel in the Boston, MetroWest Boston, and North of Boston area regularly to assess and visit clients
