    • Setting: Outpatient Rehab Center

    2. Physical therapy staff members assist with ensuring the health and well-being by providing comprehensive and quality therapy services. Come join a great team. State specific Physical Therapy license is required.

    Responsibilities:

    • Initiates skilled physical therapy treatments as per physician's orders; keeps necessary contact with physician.
    • Maintains written evaluations, progress notes, and discharge summaries for residents in physical therapy program.
    • Consults with other healthcare staff about special resident needs to improve rehabilitation or function maintenance.
    • Contributes to Care Planning meetings and attends in Director Physical Therapy's absence.
    • Keeps statistical treatment records.
    • Assists in service training programs upon request.
    • Updates and maintains the plan of care in the Inter-disciplinary Kardex.
    • Requests supplies needed for resident treatment from the Director Physical Therapy.
    • Takes over Director Physical Therapy's duties in their absence, including overseeing Physical Therapy Aides and necessary documentation.

    Qualifications:

    • Graduation from an approved school of physical therapy is necessary.
    • State license for practicing physical therapy is required.
    • Preferred two years' experience in physical therapy.
    • Interest in physical therapy treatment programs catering to geriatric, subacute, and adult populations.
    • Ability to lead others is essential.
