We provide emergency and permanent housing options and other community-based services that create opportunities for them to lead productive lives.

We operate in Los Angeles and community East and South East of Los Angeles County.


JOB TITLE:
SPA7 Short Term Housing Case Manager


The primary responsibility of the case manager is to provide case management services to youth age 18-24 that are enrolled in the SPA 7 Transitional Housing Eva's Home.

Eligible individuals will be referred by the SPA 7 Coordinated Entry System. The goal of the program is to transition youth in to permanent housing opportunities.

From time to time, the case manager may be asked by program director or executive staff to conduct activities or tasks necessary to the operations of the organization.


DUTIES AND RESPONSIBILITIES

Program:

  • Coordinate intake of eligible clients.
  • Assess clients' challenges and barriers as well as strengths.
  • Develop life skill trainings and demonstrations to clients.
  • Elaborate with client their Individualized Service Plan that will be updated monthly.
  • Provide case management services to achieve the outcomes set in the ISP.
  • Provide clients with referrals and monitor progress as needed, in particular to mental health and substance abuse services, and toward opportunities for educational & career development and personal enrichment.
  • Evaluate transportation needs and support them in developing a sustainable plan.
  • Provide clear direction, support and follow up to clients.
  • Support clients in identifying permanent housing options.
  • Develop appropriate procedures in case of crisis.
  • Coordinate other program staff in service delivery and facility monitoring.
  • Delegate activities and follow up to other program staff to maximize clients' progress.
  • Maintain appropriate and professional resident relationships with particular attention to professional boundaries.
  • Cultivate partnerships with community stakeholders and public agencies to expand opportunities for youth in the program.

Administrative:

  • Maintain files in a secure and locked cabinet.
  • Enter client data into the HMIS system on an ongoing dailytoweekly basis
  • Support development of policies and procedure for the program.
  • Collect client documentation.
  • Prepare and submit reports in a timely manner.
  • Be part of program and department meetings.

Other:

  • This position is part of a team to manage all clients in the program. While each case manager will be the primary contact for their assigned a case load, depending on program needs each case manager will be required to support the other associate as needed.
  • Follow the agency professional code of conduct at all times.
  • Be a team player and maintain effective and constructive communication within the shortterm housing team and within the agency at large.
Other duties and responsibilities may be developed by immediate supervisor according to program needs.

  • Ability to work independently and with others—including other employees, clients and members of the public—in facetoface and telephonic contexts

  • Ability to be flexible and adapt to changing work demands
  • Maintain high level of concentration and attention to detail for extended periods of time
  • Ability to respond effectively to sensitive inquiries or complaints
  • Maintain a high level of ethical and professional standards in accordance with agency and community policy

Physical Skills and Abilities:

  • Ability to talk or hear in order to give and receive information and instructions
  • Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls
