    Position Overview:


    The Program Specialist is responsible for supporting the Texas Asthma Control grant under the Asthma Control Program by providing assistance on grant-related goals, objectives, and activities, including a variety of stakeholder and community engagement initiatives.

    The specialist will be responsible for fostering partnerships, stakeholder collaboration, and strategic planning to support asthma control within the Harris County jurisdiction.

    Duties & Responsibilities:

    • Develops initiatives to fulfill all programmatic goals and objectives, including meeting grant deadlines and requirements, strategic stakeholder participation, and sustainability planning.
    • Establishes and maintains collaborative relationships with community stakeholders, schools, providers, and community healthcare agencies to facilitate and support quality asthma care.
    • Facilitates asthma self-management education (AS-ME) and home assessment sessions for community members from priority settings identified by environmental scan.
    • Recruits community leaders to serve as asthma program facilitators who will administer AS-ME programs in their respective communities.
    • Facilitates training for community health workers, healthcare providers, school nurses, and like professionals on AS-ME and quality asthma care.
    • Attends community outreach events, such as health fairs, forums, and conferences, to support programmatic goals.
    • Assists with gathering and analyzing data to be submitted for monthly and quarterly progress reports.
    • Assists with managing programmatic inventory and supplies needed for each AS-ME and home assessment cohort.
    • Performs other duties as assigned.

    Requirements


    Education:

    • High school Diploma or G.E.D equivalent

    Knowledge, Skills and Abilities:

    • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)
    • Excellent communication skills (verbal & written)
    • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment

    Experience:

    • Six (6) months of experience in community engagement, program development, or direct work under public health grants

    General Information

    Position Type and Typical Hours of Work:

    • Full-Time | Grant-Funded
    • Monday - Friday from 8:00 am to 5:00 pm
    • Must be willing to flex work hours when required, including weekends.
    • Expected to travel within Harris County jurisdiction.

    Work Environment:

    • This job operates in both a professional office and a community environment.
    • This job frequently requires visits to Harris County Public Health's various clinic sites including field operations.

    Physical Demands:

    • This role requires light physical effort; some duties require the ability to lift up to 20 pounds and walking, bending & standing for periods at a time.
