Position Summary

Administrative Clinician charged with the coordination of Medical Management, Quality Improvement, and Credentialing functions for the assigned departments, in support of the strategic plan of the system, keeping with the attendant policies and procedures. The Administrative Director of Clinical transformation and Medical Management will help with our efforts to better understand the cause of patient outliers and assist with efforts to reduce the number of outliers through improvements in care and documentation. This director will help oversee system-wide value improvement activities, efforts including developing, and implementing care plans, prioritizing the tactics and providing leadership to ensure implementation of the plan of care. S/he will oversee the coordination of care, proper clinical documentation and care plans to ensure patients' needs are met in a high quality, efficient manner.

Primary Position Responsibilities

Responsibilities:
Director to drive change for medical management and clinical transformation

Oversight of Hunterdon Medical Group Care transition team

Advanced Practice Provider Medical Director Oversight with APP's at Hunterdon Medical Center and Hunterdon Medical Group.

Oversight of APP staffing models clinical work flows and credentialing and quality
-
Gap analysis (current state, change management, actionable items, sustainability)

Dashboards, sustainable practices
-
Establish workflow, protocol and policy development
-

mitigate redundancy of orders w evidence based practice

Organizational chart creation to support accountability

Tech Stack management: work to develop infrastructure of workflow, perform gap assessment to manage upcoming future changes with EHR implementation

Medical Staff Gaps - doctorate level clinician with operational experience able to drive assessment and management of medical staff

Identify opportunities to streamline care with clinical pathways that mitigate the clinical variation of practice, develop pathways/workflows to support optimal efficiency, with attention to the clinical documentation to support level of care, and utilization of resources to ensure high quality clinical management of episode of care

Subset of Quality: clinician quality, work in collaboration with quality department to oversee FPPE/OPPE of Med staff with respective chairs, and quality support staff

Comprehensive denial management

Collaborative roles:

-
Drive clinical performance around UR/Denials Management/ medical necessity, working in collaboration with Dr. Ashraf (physician facing) to mitigate the clinical operational needs of the organization. Denial team process and trend the denials sharing with the physician director to improve the clinical documentation process.

Work with chair of MSPI to ensure peer review process being managed

Medical Records/Health Informatics CDI

Work with CMIO and innovation to develop dashboards of clinical performance to support PI initiatives for medical staff
-
Work with risk Management and department chairs to manage provider specific clarity reports

Minimum Education Requirements

Required:
Doctorate -Prepared Nurse Practitioner


Master's Degree in Healthcare or Business Administration or Doctorate

Preferred:
Doctorate -Prepared Nurse Practitioner


Minimum Years of Experience

Required: 5-7 years of progressively more responsible job-related experience of which at least 3 in upper level managerial/administrative clinical leadership position with responsibility for fiscal and staff management

Quality Improvement experience > 3 years

Preferred:


License, Registry or Certification

Knowledge, Skills and/or Abilities

Required:
Experience leading large scale organizational transformation from strategy development through execution and value realization

Experience in matrix management, including own function and providing matrix oversight to others

managing through some levels of ambiguity

Strong oral and written communication skills, can visually convey concepts

Ability to create a function "from scratch"
