JOB SUMMARY:

This role acts as a link among RCHSD members/families, medical providers, and behavioral health providers.
Create rapport and maintain effective relationships with internal and external contacts on all levels.
Communicate efficiently with individuals from various cultural and ethnic backgrounds.
Identify and overcome barriers to members' care, aid in locating resources, and stress the importance of timely care access.
Serve as the main point of contact for members/families and play a key role in their interdisciplinary care team.
Interview members to assess strengths, issues, goals, and service/resource needs, offering support in navigating the health care system.
Collaborate with the case management team to address member concerns and medical/psychosocial issues.
Efficiently manage and prioritize a caseload, maintaining accurate records and documentation.

MINIMUM QUALIFICATIONS:

Associate's Degree
2 Years of Experience
Bilingual in English/Spanish, English/Vietnamese, or English/Farsi preferred. Proficiency in spoken and written form is required.
Understanding of HMO, Medi-Cal/Medicare, and health services.
Familiarity with health care service delivery, managed care practices, Medi-Cal eligibility, and benefits.
Knowledge of techniques to serve diverse social and ethnic groups, especially the SPD population.
Ability to follow effective charting practices and guidelines.
Bilingual in English/Spanish, English/Vietnamese, or English/Farsi preferred. Proficiency in spoken and written form is required.

The expected salary range for this role is $20.00 to $27.50
