The German Pension Insurance Hessen is looking for a Prevention and Aftercare Coordinator (m/f/d) for the Rehabilitation and Clinic Management Department (Salary Group A 11 HBesG/ Pay Group 11 TV EntgO-DRV) in Frankfurt am Main. The position is to be filled as soon as possible on a full-time basis. Application deadline: Next possible starting date. Workplace: Städelstr. 28, Frankfurt am Main.

Your tasks:

  • Participation in specialist forums of the Prevention Dialogue Forum of the Hessian State Framework Agreement for the Implementation of the Prevention Act
  • Responsible participation in the initiation and approval of prevention and aftercare services
  • Responsible participation in the implementation of the prevention and rehabilitation aftercare framework concept, as well as the decisions and requirements of the committees of the German Pension Insurance Federation
  • Presentation of the prevention service RV Fit of the German Pension Insurance to rehabilitation facilities as well as other external and internal institutions
  • Contact person for rehabilitation facilities and other healthcare institutions for prevention and aftercare services
  • Independent cooperation with healthcare companies and other pension insurance carriers in the implementation of prevention services
  • Participation in internal and external project group meetings

We expect:

  • Bachelor's degree in Health Sciences, Health Promotion, Health Management, Public Health, Prevention
