Description
Providing care/ visiting individual clients in their homes. Monitoring and reporting any changes. Monitor and supervise HHA services provided to our clients.


Qualifications:

NYS Registered Nurse License. 3 years of experience in supervision, administration, and home care.


Responsibilities:

Ongoing assessment and care planning for assigned clients every six months. Making sure to maintain plans appropriate to the clients' changing needs. Assessing clients before admission, and during readmission. Determining if the services of paraprofessionals meet the client's needs. Assisting HHAs with six hours of Mandatory In-Service Training,
