About us
Health Projects Center is a 501(c)3 non-profit corporation established in 1980. We provide exceptional community-based programs that support elders to live and age with dignity at home. We deliver culturally appropriate programs in both English and Spanish for older adults, family caregivers and health workers.

Health Projects Center serves Monterey, San Benito, and Santa Cruz Counties.

We run the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program, and the Central Coast Area Health Education Center.

Our programs support over 1,300 families annually to live and age at home at a fraction of the cost of an institution.


Why This Role is Important to Us:


Essential Functions:


  • Comprehensive Intake and Screening:
  • Conduct thorough initial intake and screening processes for ECM participants, ensuring a comprehensive understanding of their unique needs and circumstances.
  • Efficient Application Processing:
  • Participant Support and Information Provision:
  • Act as a reliable source of information and assistance for participants, responding promptly to their needs or inquiries, as directed by the Director of Care Management, Lead Care Manager or Care Manager.
  • Procurement and Delivery Assistance:
  • Facilitate the purchase and delivery of items requested by the primary care manager, contributing to the seamless fulfillment of participant needs.
  • Regular Care Management Contacts:
  • Execute monthly care management contacts, providing participants with ongoing support and addressing any emerging needs or concerns, as directed by SCM or SWCM.
  • In-Home Quarterly Visits:
  • Conduct thorough Quarterly Home Visits, engaging directly with participants in the comfort of their homes to assess their living conditions and wellbeing comprehensively.
  • Electronic Record Management:
  • Meticulously document all activities in the participant's electronic record, ensuring accurate and uptodate information is accessible for effective care planning.
  • Advocacy and Community Referrals:
  • Serve as an advocate for participants, offering guidance, information, and community referrals to enhance their overall wellbeing and quality of life.

Qualifications:


  • Relevant work experience

Preferred Qualifications:


  • An Associate degree
  • A Bachelor's degree
  • Ability and willingness to work and drive in Santa Cruz, San Benito, and Monterey Counties.
  • Clean driving records.
  • Ability to navigate local community referral systems and utilizing available resources.
  • Strong organizational skills coupled with effective time management.
  • Demonstrated ability to work independently.
  • Capacity to effectively manage a hybrid work environment.
  • A positive approach to work.
  • Capacity to effectively support and collaborate with diverse clients and staff.

Physical Qualifications:


  • Comfort with driving for extended periods and navigating diverse road and weather conditions in Monterey, Santa Cruz, and San Benito Counties.
  • Ability to enter and exit vehicles multiple times a day and to drive safely and responsibly in compliance with all traffic laws and regulations.
  • Ability to stand, walk, and move around for extended periods during inhome visits and participant interactions.
  • Capability to lift and carry up to 25 pounds of equipment or materials as needed for procurement and delivery assistance.
  • Comfort with navigating various physical environments, including participant homes, to conduct quarterly visits and assessments.
  • Capacity to sit and work at a computer workstation for prolonged periods, ensuring meticulous electronic record management and documentation.
  • Adequate vision and hearing to communicate effectively with participants and colleagues, both in person and over the phone.
  • Ability to operate basic office equipment such as computers, printers, and phones to fulfill job duties effectively.

Conditions of Employment

  • Possession of a valid California driver's license and access to an insured and reliable vehicle.
  • Must pass a criminal background check.
  • COVID19 vaccination required.

Compensation and Benefits

  • Starting salary is $23.00$26.00 per hour depending upon skills and experience. We pride ourselves in maintaining a flexible and friendly work environment.

We provide comprehensive benefits including:

paid vacation (15 days); paid sick leave (80 hours); company paid medical, dental, vision, chiropractor/acupuncture, and life insurance; travel reimbursement; educational/fitness allowance; and pre-tax retirement plan with 6% of salary contributed by employer.
