Are you a compassionate and experienced Registered Nurse looking for a rewarding career helping patients in their homes? We're seeking a highly motivated RN to join our team as a UAS Assessor

In this Hybrid role, you'll split your time between working from home (2 days) and visiting patients in their homes (3 days) in the Bronx, Manhattan, and Brooklyn areas.

Responsibilities:


  • Conduct comprehensive UAS assessments in patients' homes
  • Evaluate patients' needs for personal care, medication management, and other services
  • Collaborate with the care team to develop individualized care plans
  • Document assessments accurately and efficiently
  • Provide patient education and support

Qualifications:


  • Active Registered Nurse (RN) license in New York State
  • Minimum 2 years of experience as a Registered Nurse
  • Minimum 2 years of experience in home health, home care, or longterm care settings
  • Strong assessment and communication skills
  • Compassionate and patientcentered approach
  • Proficiency in computer documentation
  • Reliable transportation (for onsite visits)
