Conduct initial assessments of patients' health status
Develop and implement individualized care plans
Administer medications and treatments as prescribed by physicians
Educate patients and their families on managing their conditions
Monitor patients' progress and adjust care plans as needed
Collaborate with multidisciplinary healthcare team members
Document all patient interactions and interventions accurately
Provide emotional support to patients and families

Qualifications and Skills:

Valid RN license in the state of practice
Minimum of one year of nursing experience, preferably in home health
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and as part of a team
CPR certification
Reliable transportation and valid driver's license
