Scope of Position In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel.
Position Summary This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas.
Duties include but are not limited to:
medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses.
Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location.Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff).
Duties and Responsibilities Dispenses medication pursuant to a physician's order according to the laws and regulations as defined by the Ohio State Board of Pharmacy and The Ohio State University Medical Center's drug distribution policies.
85% Professional Practice Activities:
Successfully be granted and maintain applicable pharmacy privileges through the OSUWMC medical staff
Utilizes pharmacy privileging regularly in providing evidence-based clinical care and elevating practice
Recommends and plans drug therapy considering clinical appropriateness, safety, efficacy, and cost
Attends providers' service rounds as applicable
Responds to drug information requests from all internal and external customers
Maintains and reviews patient profiles
Evaluates providers' orders for appropriateness, screens for drug interactions (including drug-drug, drug-herb/dietary supplement, drug-food interactions in collaboration with Registered Dieticians), and recommends alternative therapy choices when appropriate
Prevents, detects, manages, and documents adverse drug reactions to minimize adverse events in patients
Completes pharmacokinetic and therapeutic consults and recommends alternative therapy choices when appropriate
Maintains accuracy and timeliness in drug dispensing to ensure a high level of patient care and customer service
Performs patient education in collaboration with nurses, physicians, and other health care professionals
Demonstrates ability to work as a collaborative member of the team by exhibiting quality interpersonal skills, remaining flexible, and approaching work and collaborators with open-mindedness
Maintains organized workspace and productivity of self while fostering efficient operations in a dynamic and changing environment
Participates in cross-coverage of adjacent services to maintain operations

Approves new drug orders in collaboration with other pharmacists, nurses, and physicians
Fulfills all applicable Medication Use Evaluation requirements

5% Supervisory Activities:
Act as a role model for administrators, staff, and the community. Utilizes credentialing in applicable areas.
Supports team and management initiatives
Identify issues and possesses general understanding of department operations, workflow, and medication distribution; proactively identifies and resolves operational barriers.
Participates in recruitment; orients/trains new pharmacy team members
Manages personnel and supply resources to promote efficient, cost effective operations.
Contributes to the annual performance review of staff by providing objective and timely performance feedback to managers
Records and monitors Quality and Service Indicators and provides input to managers regarding quality improvement
5% Teaching Educational Activities:
Conducts experiential and didactic teaching of Pharm.
D. students, interns, externs, pharmacy residents, and regular staff; Educates medical residents, fellows, physicians, advanced providers, and students on correct drug use, prescribing, and policies; Reviews subject-specific literature and presents information during medical staff rounds
Participates in and presents departmental continuing education programs and drug inservices in addition to presentations to local community groups, the College of Pharmacy, other hospitals, pharmacy departments, and professional meetings
Provides expert consultation and education to influence optimal medication management
5% Research &
Scholarship Activities:
Manager, Assistant Director or Associate Director
Participates in research protocols pertaining to site-specific clinical activities; Develops residency research ideas
Contributes relevant publications to professional journals

Minimum Qualifications
For Hire:
Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.
D./MS) and residency; and/or equivalent experience preferred
On going:
Conducts experiential and/or didactic teaching of pharmacy students (including interns), pharmacy residents, and permanent staff. Professional instruction to the hospital and clinical staff or other professional organizations.
Participates in and presents intra-departmental continuing education programs in addition to professional presentations to local community groups, the College of Pharmacy, other hospitals and pharmacy departments, and professional meetings.

Supervises pharmacy technicians, pharmacy interns and new pharmacists training in the area. Provides information relative to staff performance for use in performance evaluations. Documents and discusses inadequate performance with the above individuals when appropriate (i.e., tardiness, improper techniques, etc.)
Contacts:
Physicians, nurses, pharmacists, pharmacy technicians, pharmacy residents and undergraduate students. Weekly contact with administrative personnel in the Pharmacy Department.
