Social Worker or Health and Nursing Professional (m/f/d) for Social Services / Discharge Management at Sana Kliniken Duisburg

We are currently seeking a Social Worker or Health and Nursing Professional (m/f/d) to join our Social Services / Discharge Management team at our acute care hospital in Branchville.

This is a full-time position (38.5 hours/week) with an indefinite contract.

Sana Kliniken Duisburg GmbH operates two hospitals: Wedau Kliniken in Duisburg-Wedau and Bertha-Krankenhaus in Duisburg-Rheinhausen. Both hospitals are modern teaching hospitals with a total of 17 departments and 678 beds, providing regional top-level care.

YOUR RESPONSIBILITIES

Providing advice, information, and support to patients, caregivers, and family members in social, economic, rehabilitative, vocational, and/or nursing matters.
