Medication History LVN - Midland, United States - Midland Health
Description
The primary duty of the medication history nurse is to obtain accurate and timely home medication lists for patients admitted to the emergency department, procedural areas, or inpatient units of the hospital. In addition, they should have the ability to perform the essential duties and responsibilities of a Registered Pharmacy Technician (RPhT).
SHIFT AND SCHEDULE
Full Time 7:00 AM – 7:00 PM
ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS
• Identify patients to be admitted from the emergency department or via direct admit
• Interview and discuss home medications with patients
• Contact family, pharmacies, or healthcare providers to complete medication history
• Identify correct drug, dose, route and frequency as well as last dose taken
• Obtain or confirm outpatient pharmacy information
• Clarify drug allergies and reactions
• Input medications and allergy information into the electronic health record
• Perform the essential duties and responsibilities of a pharmacy technician if time allows
EDUCATION AND EXPERIENCE
• Must be certified either through Pharmacy Technician Certification Board (PTCB) or National Healthcare Association (NHA) and registered in Texas as a Pharmacy Technician within two years of hire.
• Must register as a Pharmacy Technician Trainee in the state of Texas upon hire
• Must have an LVN license in the State of Texas
• Must be proficient in computer-based operations and in performing all the essential duties and responsibilities of a RPhT.
• Must have good comprehension of hospital and departmental policies and procedures
• Must fully comprehend all the roles and workflow of pharmacy technicians
• Beneficial skills: ability to manage data in spreadsheets, give effective feedback, and utilize project management techniques.

To perform this job successfully, an individual must be able to perform each essential
responsibility satisfactorily. Reasonable accommodations may be made to enable individuals

Stand, walk, sit, stoop, reach, lift, see, speak and hear. Lifting is limited to 35 lbs. for
clinical staff and to 50 lbs. for non-clinical staff. The individual must use an assisted-lift
device or get another individual(s) to assist with the lift that is over these maximum
limits.
