TEEMA Group is looking for an experienced Case Manager MSW to join our team in Inglewood, CA, USA.

The Case Manager will be responsible for providing case management services to adults with mental health and/or substance use disorders.


Responsibilities:


  • Provide case management services to patients, including assessment, planning, coordination, monitoring, and evaluation of services.
  • Develop and implement individualized service plans for clients.
  • Provide crisis intervention and support services.
  • Provide advocacy and support to clients.
  • Collaborate with other professionals to coordinate services for clients.
  • Provide referrals to community resources.
  • Guide patients towards independence through comprehensive case management.
  • Other duties as assigned

Qualifications:


  • Masters Degree in Social Work, Psychology, Counseling, or other related social service field required.
  • Minimum of 12 years of experience in social work
  • 1+ years of relevant experience in Case Management
  • Certification in Substance Abuse Counseling is highly preferred
  • Performs inhome visits, whether at projectbased units or scattered sites, to deliver case management services and ensure the wellbeing of clients.
  • Valid driver's license
