_Minimum Qualifications_

  • Bachelor's Degree in Social Work and related field
  • 1 year experience in related field
  • Have an understanding of the following concepts/models/services: strengthbased services; "core" medical/mental health conditions/challenges faced by ECM target population; social determinants of health; care management; transitional care
  • Be familiar and comfortable with the ability to work with the following target populations:
  • High utilizers: members with multiple hospital admissions, skilled nursing facility stays or frequent ER room visits that could have been avoidable
  • Adults with Serious Mental illness or substance use disorder who have cooccurring chronic health conditions and Serious Mental illness (SMI) or substance use disorder (SUD) and are actively experiencing one complex social factor (e.g. food, housing, employment insecurity).
  • Understand the importance of protecting member confidentiality/HIPPA regulations
  • Active and Current 1st Aid and CPR certification
  • Be of good moral and professional character
  • Job Duties and Responsibilities_
  • Report directly to the ECM Director
  • Be able to complete member outreach activities that include: locating members to educate them on the possible benefits of the ECM program; completing basic enrollment forms to initiate member receipt of ECM programs; and obtain/document verbal or written consent to receive ECM benefit and to share information for care management purposes to the extent required by law
  • Be able to and actively attempt to work in the "field" which is not limited to but includes: member's homes/residence; communitybased care settings
  • Complete comprehensive health assessments and home assessments as needed: to assess risks, needs, goals and preferences, and collaborate with members as part of the ECM process. Be able to integrate motivational interviewing throughout all interactions with the member
  • Able to develop resources, community networks and services that can meet the member's needs identified in Individual Plan of Care
  • Ability to help members transition safely and easily between different levels of care and delivery systems and execute needed care coordination triggered by care transitions
