Position Summary:


The Rapid Rehousing Case Manager will assist community-based households in achieving increased stability and self-reliance, the RRP Case Manager provides comprehensive casework and support services.

The RRP Case Manager is responsible for counseling and case management for households moving from homelessness, shelter or transitional living programs to permanent housing.


Responsibilities:


  • Responsible for providing case management services for newly housed families and individuals to include: crisis intervention for issues threatening physical safety and the security and stability of housing; identification and intervention in housing, personal, or other conflicts that could lead to housing; landlord/tenant relations; life skills building; helping households to develop and maintain positive and natural systems of support within their communities; and helping the households to access other resources in the community.
  • May be responsible for providing additional assistance to households including: assistance with transportation, food, clothing, furniture, and referrals to financial assistance and other programs across the community.
  • Home visits and onsite individual meetings are required and scheduled based on client need.
  • Responsible for providing support services around maintaining housing successfully.
  • Education and counseling for tenants regarding timely rent payments and fulfilling lease obligations to ensure housing retention; and early identification and assistance with issues that could threaten housing stability.
  • Maintain organized files and adhere to timely data entry into HMIS and/or other data system
  • Participate in trainings related to best practices and Coordinated Access Network system improvement including but not limited to motivational interviewing, traumainformed care, HMIS, domestic violence, etc. to ensure consistency across CAN and improved service delivery
  • Work collaboratively with the CAN providers

Qualifications:


  • Bachelor's degree and/or at least three years of experience providing homeless services
  • Knowledge of, or ability to quickly develop competency with, issues related to homelessness, effective intervention techniques, and basic understanding of mental health and substance abuse issues
  • Highly motivated, creative problem solver
  • Knowledge of community resources
  • Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and/or, other conditions or situations that have impacted housing stability
  • Strong organization skills and ability to coordinate and complete multiple tasks simultaneously
  • Excellent written and verbal communication skills, including telephone communication
  • Ability to work independently and with a group, and maintain good working relationships
  • Driver's license and daily use of a reliable and properly registered and insured motor vehicle
  • Must adhere to all professional standards and ethical practices, including creating and maintaining healthy boundaries and professional relationships with clients, coworkers, and others
  • Strong sense of responsibility and a commitment to the mission of ending homelessness

Pay:
$ $23.00 per hour
