- Provide comprehensive care and skilled nursing to patients in their home.
- Conduct assessments and under the guidance of the supervising physician, develop plans of cares for patients, utilizing nursing skills and techniques to provide ongoing and quality care to the patient.
- Deliver case management and follow up skilled nursing visits with each patient assigned.
- Engage family members and caregivers, maintaining communication about the care plan, providing training and education to ensure consistency with care.
- Assess medications to identify possible ineffective therapy or adverse reactions, side effects, drug allergies, or contraindicated medications.
- Provide ongoing communication with other members of the patents' multidisciplinary team, including Home Health Aides, Physical Therapists, Occupational Therapists and other external healthcare providers.
- Maintain thorough and accurate documentation on electronic medical records, ensuring seamless course of care and compliance with all regulatory requirements.
- Other duties as assigned
- Valid RN license in state of Louisiana
- Valid driver's license, auto insurance and reliable vehicle
- Minimum one year of clinical experience as a Registered Nurse (RN)
- Current CPR certification
- Strong attention to detail, time management and decision-making skills
- Demonstrated timely, consistent job attendance history
- Must be comfortable working in a fast-paced environment
- Commitment to patient quality as a top priority
- Flexible and proactive to meet varied patient needs
- Self-motivated and able to work autonomously to ensure completion of tasks
- Able to collaborate with other healthcare professionals
