Description
NFI Massachusetts, Inc. is currently seeking a full-time Clinical Case Manager for our Intensive Residential Treatment programs located in Westborough, MA. This is a secure residential program for adolescents who have experienced trauma.


The program provides intensive therapeutic services to clients who are referred from the Department of Mental Health after inpatient hospitalization.

Services include a multi-disciplinary team providing individual, group, family, and milieu therapy, and the accompanying case management for each client.

The Case Manager directly assists the clinicians with a variety of functions. This includes but is not limited to scheduling of treatment team meetings, communication with school districts and SEIS, collateral contact (which includes family members and DMH/DCF case workers, case-coordination, documentation, data collection.

The Case Manager also may support the residential staff in the milieu during activities or other aspects of the daily structure when requested.


Hourly:
starts at $22 an hour commensurate with experience and education


Responsibilities:


  • Coordinate treatment planning meetings for clients from preadmission to post discharge.
  • Support clinical staff with administrative tasks as assigned.
  • Participate in daytoday program structure including groups/activities when possible.
  • Attend and provide documentation for weekly TEAM meetings.
  • Attend treatment team meetings and associated documentation.
  • Participate in community meetings.
  • Coordinate with Transitional Specialist around tasks associated with discharge planning.
  • Available for crisis intervention and support in the milieu.
  • Perform physical intervention in the event of a youth crisis.

Job Requirements:


  • Three years of related experience required
  • Bachelor's degree preferred
  • High energy level, superior interpersonal skills and ability to function in a team atmosphere.
  • Must have excellent organizational skills.
  • Must hold a valid drivers license.
