The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment.

The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes.

They will also complete appropriate documentation in MS4 for inquiry calls for Outpatient. They will also assist with outcome reporting as needed. He/She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Qualifications:

Job Requirements:

Education Requirements, Bachelor's degree required, Master's degree preferred.
Minimum two (2) year of experience working in a psychiatric/mental health field setting with patients aged 1265yrs+.

Must have or able to obtain a Level 1 Fingerprint Clearance Card.
