- Conduct in-person visits within specified timeframes for eligible hospice patients and communicate findings to the Hospice Medical Director
- Collaborate with the Hospice Medical Director as necessary, update the primary physician on recommended actions, and cooperate with other medical professionals when needed.
- Develop and coordinate individualized care plans. Record issues, establish suitable goals, interventions, and the patient/family reaction to hospice care. Work in conjunction with the patient/family, primary physician, and other IDG members to deliver comprehensive care.
- Provide guidance and oversee the patient/family in self-care practices when applicable.
- Maintain precise and pertinent clinical documentation on the patient's status. Inform the Administrator and Hospice Medical Director about the patient's condition and any identified needs/challenges.
- Minimum of five years of nursing experience; with a focus on clinical nursing in oncology, hospice, geriatrics, or intensive care deemed beneficial.
- Current certification as a clinical Nurse Specialist or Nurse Practitioner in the respective state.
- Valid certification from a recognized body like ANCC, NBCHPN, or AANP.
- Active driver's license and car insurance, access to a reliable vehicle or public transport.
- Valid CPR certification essential.
- Preferably certified in Palliative Care ARNP.
- Familiarity with the interdisciplinary team approach.
