    Extraordinary advance care management group is looking to hire a Nurse Practitioner or Physician Assistant in a palliative care setting in the Southern Pines, NC area

    About the Group:

    • Providing exceptional care to those in need of hospice, palliative, home health, and personal care
    • This group has 590 locations across 38 states and 34,000 associates
    • As a team, this group is equipped to respond quickly and effectively as a patients care needs evolve with advanced tools and technology, expertly trained teams, and some of the most experienced caregivers in the US

    Your Role with the Company:

    • Full-time, 4/10s, flexible schedule, no weekends
    • Rotating 7 day phone triage call every 6-7 weeks
    • APP will work autonomously in an in home setting for palliative care seeing 3-7 patients per day
    • NP/PA will see patients within a 50 mile radius of the Southern Pines area

    Background Profile:

    • Board Certified Nurse Practitioner / NP or Physician Assistant / PA
    • 3+ years of experience as an Advanced Practice Provider required
    • Hospice, palliative care, or home health experience preferred
