Responsible for conducting career management activities, case management and providing supportive services to eligible customers.

Service being provided to clients who are eligible for assistance through the IL Department of Human Services (DHS) TNAF and SNAP Job Placement programs.


DUTIES AND RESPONSIBILITIES

  • 1. Adheres to the mission of the corporation.
  • 2. Administer program orientation, conduct intake paperwork, and perform other administrative duties.
  • 3. Interviews customers or their authorized representatives to gather information to assess service needs.
  • 4. Performs visits to customers' homes and training provider's sites as needed.
  • 5. Develops and implements service plans to meet customer needs.
  • 6. Coordinates service provider activities with additional staff, such as business service representative (BSR)
  • 7. Implements prescribed programrelated procedures and accurate case management.
  • 8. Provides ongoing case management and serves as a liaison between customers, and service providers.
  • 9. Assist customers in exploring career opportunities; including informing them of job opportunities, and job details.
  • 10. Assist customers in matching their job qualifications and skills with potential employment opportunities.
  • 11. Develop a working knowledge of available resources, including internet sites, to assist customers in identifying employment opportunities and refer eligible customers to employers and to BSR.
  • 12. Maintain customer records/case files as directed by ILDHS to document customers' progress.
  • 13. Track the number and type of employment & training activities completed with customers.
  • 14. Manages a comprehensive caseload and identifies problem areas and service gaps.
  • 15. Documents case records and prepares reports.
  • 16. Identifies and may prepare reports to supervisor on areas that are barriers to services for customers.
  • 17. Makes recommendations to management on policy and procedures changes.
  • 18. Uses personal computer to input information and maintain case reporting on various automated reporting systems used by the corporation and local Workforce System.
  • 19. Administer and grade Adult Aptitude test known as CASAS Test.
  • 20. Performs other related duties as assigned.

DESIRED KNOWLEDGE, SKILLS AND ABILITIES:


  • 1. Knowledge of specific workforce performance requirements; community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures.
  • 2. Ability to communicate effectively; to assess customer needs; to provide guidance to other staff; and to assist with management activities.
  • 3. Strong interpersonal skills, including experience working with diverse customer populations.
  • 4. Knowledge of or ability to learn and use required automated reporting systems.
  • Ability to communicate effectively with customers, vendors, administration and other employees.
  • Excellent written and oral communications skills and Bilingual in Spanish strongly preferred.

EDUCATION AND EXPERIENCE:


  • 1. Graduation from an accredited fouryear college or university with major course work in social work or a related field is generally preferred.
  • 2. An equivalent combination of experience and education may be substituted for degree.
  • 3. Experience in social work, counseling, career planning, placement, or related field.

SPECIAL REQUIREMENTS:


  • Must possess a valid Driver's License or must have access to reliable transportation in order to make required home visits.
