Resource Navigator/Case Manager
Title:
Resource Navigator/ Comprehensive Eviction Defense and Diversion (CEDD)
Reports Directly To:
Program Director
Job Summary:
To provide social services support, case management and immediate problem-solving intervention including flexible direct client financial assistance and linkages to legal services and other resources to assist in diverting evictions.
Duties & Responsibilities:
Support and development of flexible and individualized program service guides.
Facilitates linkages to a broad offering of community social services.
The supervised delivery and coordination of services to consumers include:
- Outreach case management and support.
- Referral, advocacy for linkage to essential services.
- Connection of consumer to social, spiritual, and other community resources as expressed by consumer's wishes and service plan.
Prepares documentation of work utilizing agency information systems consistent with agency/program expectations regarding timeliness and content for billing purposes and compliance.
Responsible for knowing and following agency policies and procedures.
Meets or exceeds assigned productivity standards.
Program Specific:

Case Management, which includes:
- Arrangement, coordination, monitoring and delivery of services to eligible households.
- Intake/Assessment
- Document case summary and case notes in tenant file.
- Develop an action plan to divert potential eviction.
- Collaborates with legal service partners.
- Use of the agency's resources.
- Consult with other CARC staff to determine eligibility for other programs including homelessness prevention, domestic violence, utility assistance.
- Liaison with local government and communication organizations representatives such as Board of Social Services, county housing navigators, CoCs, to identify potential resources and connect tenants.
- Tracking case outcomes (legal, diversion and housing.
- Coordinating eviction docket with court personnel.
- Liaising with court staff including the landlordtenant legal specialist, and judges at trials.
- Will be responsible to then monitor eligibility decisions rather than giving tenant contact information to do so themselves.
- Real time reporting in the Homeless Management Information system (HMIS) within 48 hours of contact.
- Report services provided, outreach, referrals generated and case outcomes.
- Coordinates with other agencies in eviction and homelessness prevention across county lines on behalf of households when necessary.
Outreach to tenant who have received eviction notices based on court listings of new eviction filings.
- Timely submission of monthly reports prior to deadlines set by grantor and/or Program Director.
- Coordination and strengthening of already existing partnerships with legal service providers and Landlord Tenant Court.
- Meets agency standards in productivity and charting compliance in HMIS.
- Completes other assignments as directed by Program Director.
Position Requirements
- Associate's Degree in Social work or other Human Servicerelated field. Bachelor's Degree preferred.
- Position requires the ability to drive. When using an agency or own vehicle, the employee must have a Valid NJ driver's license and proof of NJ Auto Insurance ($100,000$300,000). Driving will be suspended without these two forms of proof filed in Human Resources (Personnel file), annually.
- Monday
- Friday 8:30 am4:30 pm, at the Asbury Park, Freehold or Lakewood offices; in person at the Monmouth County and Ocean County Civil Courts Building/County Courthouse as scheduled by each vicinage.
Physical Requirements
Ability to work indoors at a desk for extended periods of time.
Ability to work outdoors traveling to other job sites as needed and if required.
- Listen, talk, interact, and effectively communicate with other employees, supervisors, and outside clients.
Use of phone and computer for extended periods of time.
Bending, reaching, and twisting in the performance of daily job functions.
Seeing, reading, writing, and utilizing a computer keyboard, mouse, and other computer implements.
Ability to work a set schedule consistent with the job and/or agency needs.
Responsible/Ensure compliance with CARC policies, procedures, regulatory requirements, and any other laws and regulations
