Right at Home is hiring for a Care Coordinator - Home Care Ensure clients receive the highest quality of care through performance management of caregiver staff and quality control initiatives.

This role will be responsible for conducting client care assessments/intake meetings in the field, in order to grow our new client base.

The Client Care Coordinator - Home Care will be effective at identifying needs and helping the scheduling/operations team make proper matches when identifying caregivers that can work with the client.


Essential Functions of Client Care Coordinator - Home Care:

  • Performs or supervises case management activities for individual clients, including but not limited to assessments, teaching and development and updating of the care plan or paraprofessional tasks sheet.
  • Performs successful conflict resolution/problem solving with clients and employees.
  • Performs scheduling of per diem caregiver staff on an ongoing basis. Responsible for backfilling shifts when call offs or no shows occur.
  • Documentation of care / issues/ events in WellSky and in client / caregiver files as directed by state licensure guidelines and RAH recommendations.
  • Perform new client care assessments and open new cases in the field.
  • Actively engages in training and new hire orientation in order to demonstrate professional home care standards.
  • Reviews client/caregiver notes to ensure compliance and quality in documentation and care.
  • Evaluate and supervise all Agency homecare staff both on client sites and in office for conformity with professional standards of personal and companion care, and in accordance with state health statues and home care licensure requirements.
  • Answers telephone, take inquiries or messages using good telephone technique. Uses strong inside sales techniques to turn client inquiries into new client consultations.
  • Greets and assists everyone who comes into the office in a professional and courteous manner.
  • Data entry into ClearCare (proprietary software) of all new clients, prospective clients, associates and vendors.
  • Prepares new client homecare record books, and client information packets.
  • Assists with field employee hiring orientations, inservices, disciplinary actions, etc.
  • Implements field employee safety programs.
  • Maintains compliance to all Agency clinical and administrative policies, procedures and regulatory requirements.
  • Provides for orientation and inservice education for caregivers.
  • Manages company client survey programs by promoting participation, reviewing results, and making recommendations to the management team on improving the quality of care.
  • Promotes a positive image of the Agency within the office and in the community.
  • Promotes Agency to the community, participating in community programs with other professionals when necessary and forming alliances that are most beneficial to the agency.
  • May be required to travel on company business.

Additional Functions of Client Care Coordinator - Home Care:

  • Other administrative, recruiting/HR, care management tasks as designated by President
  • Work as a backup caregiver in emergency situations (due to calloffs, no call/no show events)
  • Performs oncall/afterhours coordinator duties on a rotating basis.

Requirements of Client Care Coordinator - Home Care:

  • One year of supervisory experience preferred.
  • College degree preferred.
  • Basic computer skills (MS Office) and organizational abilities. Excellent interpersonal relations abilities. Good telephone skills.
  • Knowledge of common medical terminology.
  • Able to work independently, demonstrating sound judgment.
  • Home care or experience working in a Personal Care/Skilled Care Facility preferred.
  • Read, write speak and understand English as needed for completion of job.
  • CNA /LPN/ Rn license a plus
  • Have a valid driver's license and use of an insured automobile or access to adequate transportation.

Pay:
$1, $1,100.00 per week
