Description

Catholic Health Services of Long Island (CHS) is an integrated health care delivery system with some of the region's facilities including six hospitals, three skilled nursing facilities, and a regional home health service.


The CHS delivery system includes CHS Physician Partners, an integrated provider network of over 1,500 physicians which align closely with the health system.

CHS Physician Partners represents the population health vehicle for CHS which will position the health system for success as reimbursement paradigms shift from fee-for-service to those which are value-based.

The Provider Engagement Manager (PEM) is the face of CHS for our physician network.

The PEM will coordinate a broad scope of interventions and transformation initiatives across the CHS physician network to enable providers to succeed under value-based care.

The PEM will be responsible for building relationships with physician practices across the network and collaborating with stakeholders to introduce performance improvement interventions which enhance quality and reduce cost of care.

Interventions include enhancing workflows to improve access, introducing system resources such as home care, facilitating technology solutions, and supporting avoidable emergency room use through urgent care.

The PEM will be responsible for the performance of assigned practices across major payor categories including Medicare, Medicaid and Commercial Plans.

The successful PEM will need excellent interpersonal skills, analytical abilities, and a deep passion for helping providers keep their populations healthy.

The ability to interface with executive management will be a critical success factor.


DUTIES/RESPONSIBILITIES

  • Establish strong relationships and collaborate with office staff in implementing performance improvement processes
  • Recommend interventions which support the activities of a clinically integrated network
  • Educate provider stakeholders on incentive programs, quality measures and clinical performance data
  • Interface with hospital Chief Administrative Officers and hospital leadership regularly on network performance in given territories
  • Assist practices in transformation initiatives including Patient Centered Medical Home, connectivity to Health Information Exchanges, and behavioral health integration
  • Understand the mechanics of valuebased contracts and drive associated participation
  • Quarterback clinical and process interventions on closing gaps in care
  • Perform site visits to conduct outreach, education, training and orientation sessions for providers and key administrative staff
  • Extract relevant data and reporting material from EMR systems of partner organizations
  • Support the resolution of claims issues

POSITION REQUIREMENTS AND QUALIFICATIONS:


Skills and Experience:


  • Bachelor's degree required; Master's from an accredited university in Business Administration, Health Care Administration or other related field preferred
  • Three to five years of experience preferred preferable a project management or customer service positions
  • Excellent written and oral communication skills
  • Advanced knowledge of Microsoft Office
  • Ability to learn and navigate diverse EMR systems
  • Strong organizational, problem solving and decision making skills
  • Ability to prioritize and manage multiple projects, activities and issues effectively and simultaneously
  • Selfmotivated and selfstarter who can work well under mínimal supervision
  • Strong attention to detail, research and follow up skills
  • Outstanding customer service and negotiation skills
  • Solid knowledge and understanding of DSRIP and/or managed care industry and various provider payment methodologies
  • Ability to work both independently and in a team setting
  • PCMH Knowledge preferred
  • Valid driver's license and reliable insured automobile required. Must be able to travel locally up to 75% of the time
