- Serves as primary participant navigator and primary informational point for Community Health program participants for YMCA's evidence based programs.
- Models relationship-building skills in all interactions. Develops and maintains collaborative relationships with community organizations. Maintains regular, clear, and concise communication within area of responsibility.
- Supervises Referral Management team (Community Health Administrators), workflow and processes to ensure effectiveness and quality for enrollment and data development platform.
- Facilitates classroom delivery (in-person and virtual) of Community Health programs according to training and as needed to achieve program deliverables.
- Enrolls participants in each program and assists the Health Coach in obtaining necessary paperwork to determine program fees and establish membership benefits.
- Provides direct support to part time staff including all facilitators and health coaches for Community Health programs.
- Collaborates with community health partners to create virtual healthy living content and programing.
- Coordinates and sets vision for production of a robust holistic catalog of healthy living programming. Including: virtual wellness, fitness, mental health and well- beings series, cooking classes and nutrition education.
- Builds actions plans and strategy with Community Health Data Manager to review analytics of virtual content to determine opportunities for improvement to ensure quality and fidelity are met for each evidence-based health initiative.
- Supports and enhances strategies around participant engagement and membership conversion of all Community Health participants.
- Conducts intake appointments (using Listen First skills) to learn about members, assess readiness based on ability and willingness to change, and track participants on waitlist for various Community Health programs.
- Creates workflow and vision for new development opportunities of a comprehensive referral network and implementation of a strategy to integrate our community health programs into EMRs utilized by healthcare providers.
- Communicates with the Branch Directors, program directors, coordinators and Health Coaches as necessary with program issues, personnel conflicts, and member concerns.
- Collaborates with department leads to capture and share participant success stories within the YMCA and also with community health partners.
- Serves as community liaison between the YMCA and community partners, attending community stakeholder meetings when necessary and appropriate. Special attention should be placed on medical providers, health departments, state agencies, and like-minded community based organizations.
- Remains vigilant surrounding future growth opportunities while ensuring that all new efforts align with community needs and YMCA strategic priorities.
- Coordinates Regional Meetings for both in-person and virtual evidence-based programs.
- Works with compliance officer to ensure the program and staff adhere to YMCA of the Triangle's HIPAA policies and procedures, CDC and other relevant polices/priorities of partner organizations.
- Attend Session 0 for program overview and ensure enrollment is complete. Regularly conducts quality assurance visits to of all evidence based programs to ensure program fidelity and integrity. Monitor weekly outcomes in electronic health record.
- Builds reports of all pre and post data evaluation on patient outcomes to share with relevant stake holders.
- Partners with MIQ teams and Community Health Director to achieve a 60% conversion rate for program participants into long term members through multiple avenues of engagement. Continues to refine and implement process of involving Membership Engagement & Welcome Center staff into program delivery to increase the opportunities for relationship building.
- Provides leadership to Instructors and Health Coaches to help expand our Community Health programs across the assigned service area.
- Recruits and hires diverse staff and volunteer teams across multiple dimensions of diversity. Supports with on-boarding and developing them for success.
- Monitors & manages the hours and wages of hourly program staff according to the department's budget.
- Assists with Annual Campaign, as appropriate.
- Responsible for other opportunities as assigned by the Association Director of Community Health.
- Bachelor's degree in Exercise Science, Health Education or Public Health with 5-7 years of experience working in the community health field, preferred. Or relevant work experience in the field required.
- Achieves and maintains master level trainer within the first year of work for at least 1 or 2 Community Health YMCA evidence-based program certifications, as required.
- Strong interpersonal skills, including communication (written and verbal), decision-making and conflict resolution.
- Previous experience working with people in a healthcare or exercise setting, preferred.
- Proficiency in excel for reporting and evaluation of programs.
- Knowledge in nutrition education or behavioral and mental health support, preferred.
- Must have the ability to build strong relationships, inspire confidence, and work with diverse populations.
- Be passionate for service and improving the health of our community.
- Possess strong interpersonal and relational skills, be able to handle confidential medical records, be detail oriented and have exceptional organizational and communication skills.
- Ability to relate effectively to diverse groups of people from all social and economic segments of the community.
- Multi-lingual desired but not required.
- Basic computer skills, including Microsoft Office applications.
- While performing the duties of this job, the employee is regularly required to use a computer for extended periods of time and be able to communicate using a computer and phone/smart device.
- The employee frequently is required to sit, stand and reach, and must be able to move around the work environment.
- The employee must occasionally lift and/or move up to 20 pounds.
- Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust.
- The noise level in the work environment is usually moderate.

POSITION SUMMARY:
Under the supervision of the Association Director of Community Health, the Community Health Coordinator is responsible for managing participant interaction, daily operations and service delivery of evidence-based programming. This position primarily provides support and guidance to program participants and staff, assists in coordinating program logistics and ensures that each program achieves quality performance outcomes. There is a primary focus on working with health seekers and Health Coaches/trainers. This position must perform program responsibilities in accordance with commitments made to funders, partner organizations and the policies, goals, values, and mission established by the YMCA of the Triangle.
