Constellation is in search of a Per Diem Chaplain to join our hospice program. Our organization is dedicated to providing exceptional care and support to individuals and families in need. What We're Looking For:

• Graduate of an accredited seminary or school of theology
• Experience in end-of-life care and working with individuals/families
• Comfortable in diverse and interfaith environments
• Knowledge and commitment to hospice philosophy of care
• Ability to collaborate with interdisciplinary teams
• Previous hospice experience is a plus
• Reliable transportation for in-home patient care

Your Responsibilities Will Include:

• Conducting spiritual assessments and providing care to patients and families
• Collaborating with community chaplains and counselors
• Documenting spiritual care services and activities
• Participating in patient care conferences and team meetings
• Recruiting and supervising spiritual care volunteers
• Contributing to quality assessment and improvement initiatives
• Being available for on-call duties as needed
