    Job Title: Community Engagement Specialist at WellBe Senior Medical

    WellBe Introduction

    The WellBe care model, led by Physicians, focuses on providing advanced geriatric care to the frail, poly-chronic, and elderly Medicare Advantage patients. The goal is to improve the quality of care for a typically underserved population facing challenges in accessing healthcare. WellBe brings the care to the patient, offering a continuum of services from chronic to urgent care at the patient's home, hospital, skilled nursing facility, and more.

    General Summary

    WellBe Senior Medical is looking for passionate individuals to join the Market Based Engagement Team. As a Community Engagement Specialist, you will develop and execute innovative strategies to promote the value of WellBe Senior Medical to attributed Medicare patients. Through in-person, telephonic outreach, and community events, you will build trust-based relationships with patients, encouraging their engagement with WellBe Senior Medical. This role also involves promoting WellBe Senior Medical in various healthcare settings and patient homes.

    Responsibilities

    • Identify creative enrollment strategies for patient engagement with WellBe.
    • Engage patients through outbound telephonic outreach.
    • Enhance patient awareness of WellBe through grassroots activities and community events.
    • Develop outreach strategies to maximize patient engagement with WellBe.
    • Achieve monthly engagement targets.
    • Report daily, document outreach activities, and manage patient pipeline efficiently.
    • Address patient concerns and schedule initial provider visits.
    • Follow up with patients to ensure attendance to scheduled visits.
    • Deliver exceptional customer service with compassion.

    Job Requirements

    Educational Requirements:

    • High School Diploma/GED required; Bachelor's Degree preferred.

    Required Skills and Abilities

    • 2+ years of experience in sales, marketing, or customer service, healthcare experience is a plus.
    • Strong communication and emotional intelligence skills focused on improving patients' lives.
    • Ability to work independently, solve problems, and manage time effectively.
    • Proven track record in goal achievement and driven by performance-based incentives.
    • Proficient in Microsoft Office with excellent written and verbal communication skills.
    • Access to daily transportation for job functions.
    • Experience with CRM systems is advantageous.

    Supervisory Responsibility:

    • None

    Travel Requirements:

    • Travel locally up to 75%.

    Work Conditions:

    • Hybrid role involving office and fieldwork.
