Job Title: Social Worker at Bon Secours Mercy Health

Primary Function/General Purpose of Position

The Social Worker offers direct patient care under the guidance of a designated healthcare professional, following all regulations and guidelines of Bon Secours Mercy Health (BSMH).

The responsibilities may include providing individual and family therapy, group facilitation, discharge planning, community resource collaboration, psychosocial assessments, substance use disorder screenings, crisis assessment, and referral services.

Essential Job Functions

Individual and family psychotherapy
Group facilitation and participation in family and treatment team meetings
Discharge planning and community resource knowledge
Psychosocial assessments and crisis assessments
Substance use disorder screenings and discharge planning
Collaboration with community organizations
Completing Preadmission Screening and Resident Review (PASRR)
Post-discharge calls and participation in BSMH training
Conducting screenings such as PHQ-9, C-SSRS, and social determinants of health (SDOH)

Licensing/Certification

Licensed Social Work (LSW) (preferred, required within 6 months of hire)
Licensed Masters Social Worker (LMSW) (preferred)

Education

Bachelors, Social Work (required)
Masters, Social Work or Social Administration (preferred)

Work Experience

1 year in a social or human services setting (required)

Working Conditions

Potential exposure to high stress and fluctuating workloads
May encounter physical altercations and verbal abuse
Use of physical restraints may be necessary
Exposure to various environmental conditions

Skills

De-escalation techniques and physical interventions
Motivational interviewing and patient-centered care
Care planning, safety screenings, and collaborating within a team
Community collaboration and technology proficiency
Documentation and basic data analytics
