An established home health agency is seeking a Clinical Manager.

Must be a Registered Nurse.
The Home Health Clinical Manager is a qualified individual that provides oversight of all patient care services and personnel.

Oversight must include making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care.

The Clinical Manager participates in the development of orientation and educational programs to assist staff to continually improve patient care.


  • Provides oversight and participation in orientation, competencies, and educational programs as assigned.
  • Demonstrates a working knowledge of Home Health Quality Assessment and Performance Improvement, Oasis Data Set and Medicare Conditions of Participation.
  • Reviews referrals and coordinates staff assignments assuring that patient needs and timeliness standards are met.
  • Demonstrates knowledge and expertise in clinical decision making to meet the needs of the patient population, assuring that patient needs are continually assessed.
  • Assures interdisciplinary communication during case conferences, including appropriate documentation and followup.
  • Ensures the coordination of educational programs for staff that effectively addresses areas of performance improvement.
  • Assures the development, implementation, and updates to the patient's individualized plan of care.
  • Complete employee evaluations in a timely and accurate manner ensuring that annual supervisory visit is completed.
  • Responsible for hiring recommendations, supervising, counseling, disciplinary action and documentation for all assigned staff.
  • Counsels, disciplines and /or mentors staff in an effective and timely manner, following appropriate HR polices.
  • Completes and approves payroll and mileage for all assigned employees.
  • Assists with afterhours call for the agency. Demonstrates oversight of staff scheduling to ensure adequate staffing to address patient needs.
  • Other duties as assigned.

Requirements:


  • Resides in Southern California.
  • Current Registered Nurses Licensure in the State of California.
years supervisory experience in a healthcare setting and previous Home Health experience preferred.

  • Prior experience in supervisory role required.
  • Detail oriented with the ability to multitask and problem solve.
  • Excellent selfdirected time management and critical thinking skills.
  • Current California driver's license and proof of insurance.
  • Travel required.

Pay:
$ $67.17 per hour

Expected hours: 40 per week
