Qualifications:


Education:
Graduate of accredited school of nursing

Certification/Licensure:
Valid, current Pennsylvania state license to practice professional/practical nursing

Experience:
Previous experience in a clinic setting preferred

General Summary of Duties:

Provides professional nursing care under supervision for clinic patients in Transition of Care (TOC); triage communications between providers, patients, and office staff; address documents and update patient electronic health records and providers appropriately.


Supervision Received:
Reports directly Nursing Administration


Supervision Exercised:
None


Typical Physical Demands:
Requires vision correctable to 20/20 and hearing to normal range. The ability to work at a computer for extended periods of time.


Typical Working Condition:
Computer and telephonic communication with patients, providers, and office staff.


Expectations of Position - to include but not limited to:

  • This job requires access to PHI. HIPAA training is required and HIPAA compliance is expected.
  • Follow procedures according to OSHA.
  • Follow procedures according to FPC protocol.
  • Knowledge of the principals and skills needed for professional nursing to provide patient care and treatment.
  • Knowledge of examination, diagnostic and treatment room procedures.
  • Knowledge of medications and their effects on patients.
  • Knowledge of patient education principals to develop a selfcare program.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Skill in administering a variety of treatments and medications as directed.
  • Demonstrates ability to be confident, personable, selfmotivated and function with tact and poise in all situations.
  • Demonstrates the capability of making rapid and clinically accurate decisions based on evidence based practice.
  • Demonstrates basic computer skills.
  • Skill in recording and writing reports.
  • Skills in establishing and maintaining effective working relationships with patients, medical staff, employees and the general public.
  • Ability to maintain quality control standards.
  • Ability to recognize problems and recommend solutions.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to communicate clearly.
  • Ability to follow oral and written directions.

Examples of Duties - to include but not limited to:

  • Performs general nursing assessment of patients. Reinforces prescribed medication regimes and treatments in accordance with nursing standards.
  • Observes, records and reports patient's condition and reaction to drugs and treatments to the physician.
  • Schedules appointments and ensures preferences are given to patients in emergency situations.
  • Screens patients for appropriate information. Instructs patients in collection of samples and tests as necessary.
  • Coordinates patient care with other providers, case managers, and ancillary services.
  • Responds to and directs incoming phone calls. Instructs patient and family regarding medication and treatment instruction. Instructs patients in selfcare programs and participates in all aspects of patient education.
  • Updates, reviews and prepares patient charts.
  • Attends required meetings and participates in committees as requested.
  • Participates in professional development activities and maintains professional affiliations.
  • Maintains patient confidentiality.
  • Performs related work as required following all established company protocols.
  • Responsible for improving the overall quality and completeness of clinical documentation.
  • Ensures the accuracy and completeness of clinical information used for measuring and reporting physician and medical center outcomes.
  • Educates members of the health care team on an ongoing basis.
  • Outstanding written, verbal, telephone and interpersonal communication skills including time management and prioritization of tasks.
  • Integrate professional nursing theory into telehealth and onsite nursing practice to effectively manage patients over the phone, in the home or community; utilize judgment and best practice decisionmaking skills in telehealth nursing
