- Participates in development of protocols, policies, and practices that improve the effectiveness and efficiency of the program; contributes to opportunities to expand services.
- Participates in recruitment, hiring, training, supervising, and evaluating staff to promote the mission of the department.
- Collaborates with Compliance Officer and senior management to maintain program compliance. Performs program audits to ensure all established requirements and guidelines are met.
- Works cooperatively with providers to coordinate the care of patients with specific diagnoses by identifying problems/barriers, appropriate actions/steps and timelines for completing health care goals.
- Compiles monthly and quarterly reports, to include a narrative and analysis of statistics for contributing to quality measures and program reporting requirements.
- Establishes and maintains community partnerships to improve community health.
- Serves as a liaison with community partners who assist with program delivery services; represent the department at meetings and events as needed.

Description
Under general supervision, performs administrative functions and coordinates the assigned program for Community Health Center of Yavapai (CHCY). Education:
High school diploma or equivalent.
Experience:
A minimum of three (3) years' experience in the medical field with two (2) years' experience in a supervisory role.
Additional Requirements:
Must possess a valid Arizona driver's license.
