    This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30, 2024 or until all positions are filled.

    The initial cut-off date for referral of eligible applications will be January 28, 2024, with subsequent cut-off dates every two weeks.

    Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled.

    The following duties are required of the position; they include, but are not limited to:


    • Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the Nurse Practitioner's obligation to provide optimal care (ANA Code of Ethics for Nurses).
    • Plans and delivers technically competent care to patients.
    • Formulates a plan of care based on nursing assessments based on the ANA Standards of Nursing Practice and seeks assistance as needed: 1) Completes initial assessment of patient condition and response to treatment and initiates follow-up as needed. 2) Includes the patient and family/significant other in developing the initial plan of care and establishing goals. 3) Updates plan of care with assistance as needed. 4) Assesses patient's learning needs, learning preference, and readiness to learn. 5) Develops and implements a plan to meet learning needs, instructing patients and families in basic health teaching and discharge planning.
    • Documents the nursing process: 1) Documentation is consistent, accurate, concise, and timely. 2) Evaluates and documents patient response to interventions and progress towards attainment of goals and expected outcomes. 3) Revises plan of care according to evaluations of patient responses. 4) Uses the Computerized Patient Record System (CPRS) and the Bar-Coded Medication Administration (BCMA) software per policy.
    • Manages nursing care of patients: 1) Organizes and completes own assignments in a safe and timely manner, assisting team members as necessary. 2) Assigns patient care responsibilities based upon patient needs and competencies of staff. 3) Provides direction for team members in meeting patient care needs. 4) Follows-up to ensure that assigned delegated tasks are completed by team members.
    • Demonstrates grade-appropriate and consistent critical thinking skills: 1) The Nurse Practitioner can recognize the patient has a problem. 2) The Nurse Practitioner can manage the problem safely and effectively, recognizing his/her scope of practice. 3) The Nurse Practitioner has a relative sense of urgency. 4) The Nurse Practitioner takes the right action for the right reason (del Bueno).
    • Provides coordination between the patient and transplant centers under the direction of the supervising physician in each area (example: Gastroenterology Liver, Pulmonary Lung, Nephrology Kidney, Heme/Onc bone marrow).
    • Responsible for the logistical coordination of all transplant cases between Hampton VA Medical Center (HVAMC) and transplant centers providing effective communication and leadership with coworkers, physician, and hospital staff throughout the donation process.
    • Work with specialty specific colleagues and evaluates referrals for organ and tissue donor suitability based on Organ Procurement and Transplant Network/United Network for Organ Sharing (OPTN/UNOS) and Center for Disease Control (CDC) guidelines/recommendations and other regulatory requirements: 1) Is familiar with the processes and criteria for transplant, emergency transplant referral, and resubmission/second opinions. 2) Works with the team to make sure the referral checklist is complete. 3) Works with the team to make sure the VACO CPRS template is updated. 4) Gathers documents for supplemental referral to be uploaded in Tracer. 5) Monitors referrals for decisions and maintains communication with the team and patient.
    • Monitors patients that are transferring between one VA transplant Center to another and/or veterans transferring care from/to Hampton VAMC.
    • Maintains a permanent record of the organ and tissue donation in accordance with accepted confidentiality standards.
    • Completes the donor Electronic Medical Record (EMR) record in a timely and accurate manner.
    • Identifies organ and tissue-specific and behavioral risk factors utilizing all available resources.
    • Participate in periodic national teleconferences.
    • Facilitates adherence to VHA Directives regarding Solid Organ and Bone Marrow Transplant (VHA Directive , LVAD/Heart (VHA Directive and Organ, Tissue, and Eye Donation (VHA or other policies related to transplant care.

    Qualifications


    Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.


    Basic Requirements:

    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
    • Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE).
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
    • Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.

    Grade Determinations:
    The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:


    • Nurse I Level III - Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    • Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
    • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

    Note regarding MSN degrees:

    If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

    Physical Requirements:
    This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided.

    The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings.

    The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.

    The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

    Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.

    The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.

    Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
