    Summary


    The Hampton VA Medical Center is looking for a Physician to join as a Nurse Practitioner (Transplant) in our team. As a Nurse Practitioner, you will work alongside the Patient Care Services team to support Veterans and their loved ones.     Duties


    • Responsible for individual nursing practice and appropriate task delegation following the Nurse Practitioner's commitment to deliver optimal care.
    • Plans and provides skilled care to patients.
    • Performs procedures accurately and seeks assistance when necessary.
    • Safely utilizes patient care equipment and reports maintenance needs.
    • Identifies subtle signs of patient condition changes and responds accordingly.
    • Assigns tasks and guides unit staff activities effectively.
    • Develops care plans based on assessments, involving patients and families in goal setting.
    • Documents nursing process accurately and in a timely manner.
    • Manages patient care, assigns responsibilities based on needs, and supervises team members.
    • Demonstrates consistent critical thinking skills at an appropriate level.
    • Coordinates with transplant centers under physician supervision.
    • Facilitates transplant case coordination between VA Medical Center and transplant centers.
    • Evaluates referrals for donor suitability based on guidelines and regulatory requirements.
    • Monitors patient transfers between VA centers and maintains donation records.
    • Completes donor medical records accurately and timely.
    • Identifies organ and tissue-specific risk factors utilizing available resources.
    • Participates in national teleconferences periodically.
    • Ensures adherence to VHA Directives related to transplant care.
    • Other duties as assigned.
