- Massachusetts RN Licensure
- Valid state issued Driver's License
- One (1) year of acute medical/surgical nursing experience
- Cardiopulmonary Resuscitation (CPR) Certification
- Conducts sensitive assessments of patients' physical and psychosocial needs
- Evaluates patient/family learning needs and adjusts information delivery accordingly
- Collaborates with patients, families, and healthcare team members to establish goals and treatment plans
- Adapts to new situations smoothly
- Collaborates with healthcare providers and community resources in care planning and discharge
- Completes necessary documentation and medication reconciliation
- Manages caseload effectively within a team-based care model
- Prioritizes patient visits, messages, and phone calls promptly

Job Summary
The Home Health Registered Nurse will be responsible for delivering comprehensive home health services to clients in their residences using the nursing process. This includes assessments, interventions, and supportive care. Additionally, the role may involve telephone triage, problem-solving, advocacy, and support for patients and caregivers, with a focus on preventing hospitalization. The nurse will collaborate effectively with patients, families, colleagues, community resources, and physicians to ensure safe and efficient patient care.
Location: Malden, MA
Hours: Full-Time, Days, 40 Hours
