The Program Manager for Trans Services will be responsible for the day-to-day management, coordination, and oversight of the San Francisco Transgender Alliance for Health Resources (STAHR) and the H.O.T.

Program.

They will also be responsible for supervising and supporting case managers and staff in both programs but not limited to assisting the drop-in services offered by Trans Services.

The Program Manager for Trans Services will report to the Associate Director of Trans Services.


San Francisco Transgender Alliance Health Resources (STAHR) is a multi-agency collaboration focused on addressing the sexual health needs of trans women of color in San Francisco.

This program is responsible for leading, co-creating, and managing a multi-service, comprehensive, and seamless HIV, STD, HCV trans health initiative and access to all trans health services.


The House of Thrive (H.O.T.) Program is an innovative, multi-leveled intervention designed to build life skills, increase education and employability, and build healthier, more self-directive futures for low-income and marginalized trans people in SF.


The Program Manager will oversee the implementation of both programs, reach its benchmarks and deliverables, and evaluate its impact and results to the greater transgender community in San Francisco.


Essential Duties:


  • Lead, manage, support and supervise a team of program staff of the STAHR and HOT Programs around job performance, welldefined goals of delivery, development and implementation of staff and program work plans, staff morale maintenance, problemsolving and any program and staff related issues.
  • Serve as daytoday contact person and be responsible for overall management and coordination of the said programs. Take lead in the daytoday operations of STAHR and H.O.T Program.
  • Work collaboratively with Associate Director of Trans Services to support, review, and implement contract goals, program objectives, program budget and design strategies as needed to improve service delivery and ensure deliverables and deadlines are met in a timely fashion through creation, organization, and implementation of project timelines, calendars and schedules.
  • Oversee and implement timely data collection, data entry and maintenance, and welltimed submission of documents such as monthly, quarterly, and yearly reports and track client enrollment/activities/deliverables and contractual program documentation and reporting requirements.
  • Manage, evaluate and hire case managers and program specialists to ensure that all case management and programmatic objectives are being met with high quality engagement and are aligned with the program's overall mission and goals.
  • Provide and support high caliber case management and advocacy services to clients, engage in followup conversations and referrals that are resultoriented to additional services as necessary.
  • Assist staff with crisis management and deescalation as needed.
  • Lead, develop, distribute and provide outreach information for the programs and clinic, including but not limited to pamphlets, brochures, ads, website information, and social media posts.
  • As lead organization of STAHR, the Program Manager will be responsible for the coordination with other organizations for monthly meetings, oneonone programmatic consultation/ support, troubleshoot and any assistance needed.
  • Actively participate and engage as a representative of the agency and develop external community partnerships with various trans services that are relevant, strategic, and aligned with mission of the programs and clinic.
  • Develop, discover and attend community events in order to promote coverage options and the mission and services of the organization and clinic.
  • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Participate in required department, interdepartmental and agencywide meetings, trainings, and activities, which may include some evening and weekend hours.
  • Other duties and responsibilities as assigned.

Minimum Qualifications:


  • 35 years of work experience in case management, providing counseling, advocacy and education.
  • 13 years' experience managing contracts and staff and implementing programs.
  • Working experience with the Trans and GNC communities.
  • Bachelor's degree or a combination of experience and education will be considered.
  • Knowledge of enrollment and eligibility with public benefits and health insurance programs in San Francisco and/or California.
  • Experience and comfort in conducting outreach in various types of settings.
  • Experience working with multiplydiagnosed patients (HIV, mental health illness and substance abuse).
  • Ability to maintain confidentiality and privacy of persons, documents, data, and communications.
