Duties:


The CERS program is an essential and critical component of providing safe temporary shelter and supportive services for homeless Veterans.

VA facilities must ensure that Veterans in community-based funded programs are receiving quality services.

The incumbent is responsible for the oversight of these services as well as the case management of the Veterans who receive them.

Duties of this position include, but are not limited too:


  • Monitoring the clinical care and assesses the compliance of the programs receiving payments as described in the recipient's contract.
  • Provides oversight of the CERSfunded programs as defined by VA policy and Directive Contract Emergency Residential Services. This includes but is not limited to verification of Veteran status and eligibility for participation in programming, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, and collecting and submitting 2 participant information as required by the Homeless Operations and Management System (HOMES). Incumbent approves payments.
  • Coordinates and participates in program site inspections utilizing central office guidance and policy.
  • Participates in conference calls and attends training offered by the HCHV program.
  • Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
  • When the CERS program is located in the community, the incumbent will be required to perform most (>50%) of their job duties in the community and will be required to utilize a GSA vehicle while performing such duties.
  • Meet the needs of customers while supporting VA missions. Consistently communicates and treats customers in a courteous tactful and respectful manner. The incumbent is able to provide the customer with consistent information according to established policies and procedures. The incumbent handles conflicts and problems in dealing with people constructively and appropriately.
  • Participates in the teaching of graduate social workers and students of other disciplines as appropriate.
  • Independently provides consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans.
  • Has general knowledge of Veterans' benefits and services relating to special programs, serviceconnected compensation and nonservice connected pension. The incumbent facilitates referrals based upon Veterans' needs and eligibility.
  • Advanced knowledge and skill in the use of wordprocessing, data management and other computer systems, especially those programs in use by VHA such as HOMES and VSSC.
  • Appropriately documents in the patient's medical record in accordance with medical center policies and SOPs, and the requirements of the Joint Commission.
  • The HCHV-CERS Coordinator will obtain a Contracting Officer's Representative (COR) 1 Certification within 2 months of start and COR 2 certification within 6 months of start. The incumbent will then maintain the COR 2 certification.
  • Performs other related duties as assigned.
