We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment needs from both the client and professional's point of view.

Responsibilities of the home care nurse include, but are not limited to the following:

  • Assist with patient's ADL's and IADL's, such as bathing, grooming, meal prep, light housekeeping.
  • Monitor patient's condition, reporting any change in status to the Supervising clinician.
  • Electronic charting using Axxess.
  • Foster a supportive and compassionate environment to care for patients and assist their families.
  • Expand knowledge and capabilities by completing assigned In-Services.
  • Must have completed approved course of education to include 75 hours of training as outlined by the State of Delaware
  • Must have verifiable experience providing care in the home setting, to including, bathing, grooming, meal prep, etc.
  • Cannot have a break in service for greater than two years.
