- Research the community for available resources to meet identified needs and inform members of options
- Facilitate meetings and program visits to assist with member/family choices
- Review, monitor, and evaluate individual support plans within mandated timelines
- Develop and draft PCSP, document progress and concerns, and make referrals to community agencies
- Conduct on-site visits to members/families' homes, schools, and program sites to discuss service and support needs
- Understanding of principles and practices of case management, applied behavioral interventions, and child development
- Familiarity with methods, procedures, and techniques for developing individual support plans
- Knowledge of community resources, medical and behavioral health issues, and terminology
- Ability to develop, evaluate, and maintain member service plans
- Skills in interviewing, counseling, and working with members and family members
- The capacity to work independently, lead diverse teams, and gather accurate data from various sources
- Ability to lead a team of support coordinators in the supervisor's absence
- Arizona licensed registered nurse or a degree in social work, psychology, special education, or counseling with relevant experience
- Successful completion of background and reference checks
- Compliance with driving requirements if necessary for the role
- Completion of the Electronic Employment Eligibility Verification Program (E-Verify) for new State employees

The Department of Economic Security is in need of an experienced and highly motivated individual to join as a Case Manager Lead.
