Certified Case Manager (CCM)
Position Summary
The Certified Case Manager (CCM) provides comprehensive, non-clinical case management services to a military population, coordinating care across multiple healthcare systems. The CCM serves as a dedicated advocate and point of contact for service members, facilitating access to medical, behavioral health, and rehabilitative services while navigating complex military, federal, and civilian healthcare networks. This role requires a proactive, organized professional with strong communication skills and a deep commitment to supporting the health and wellness of those who serve.
Key Responsibilities
Case Management & Care Coordination
- Develop, implement, and monitor individualized care plans to facilitate access to appropriate healthcare services for assigned clients.
- Coordinate care between military medical providers, the Department of Veterans Affairs (VA), TRICARE, and civilian healthcare networks.
- Facilitate timely referrals and appointments, ensuring clients receive necessary medical, mental health, and rehabilitative services.
- Track and monitor client progress through medical evaluation and disability determination processes.
- Serve as a primary advocate for clients navigating complex healthcare and benefits systems.
Documentation & Reporting
- Maintain accurate, thorough, and timely case management records in full compliance with applicable privacy and regulatory standards, including HIPAA.
- Prepare and submit regular case status reports to appropriate supervisory and administrative personnel.
- Document all case activities, interventions, referrals, and follow-up actions in designated systems.
- Coordinate with administrative support staff on proper filing, retrieval, and handling of client medical documentation.
Liaison & Coordination
- Liaise with medical officers, behavioral health professionals, and administrative personnel to ensure seamless client support.
- Coordinate with VA representatives, TRICARE case managers, and community-based organizations to facilitate continuity of care.
- Attend case conferences, medical review boards, and relevant team meetings.
- Travel to various client meeting locations and facilities within the state as required.
Required Qualifications
Certification & Licensure
- Active, valid Certified Case Manager (CCM) credential issued by the Commission for Case Manager Certification (CCMC) — required.
- Active Registered Nurse (RN) licensure in the State of Maine, or eligibility for licensure by endorsement.
- All credentials must remain in good standing throughout employment.
Education
- Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing, Healthcare Administration, or a related field preferred.
Experience
- Minimum three (3) years of case management experience in a healthcare, military, or government setting.
- Experience with military healthcare systems (TRICARE, Military Treatment Facilities) strongly preferred.
- Familiarity with VA benefits programs and the Transition Assistance Program (TAP) strongly preferred.
- Prior military service or experience working with military populations highly desirable.
Other Requirements
- U.S. citizenship required; must be able to obtain and maintain necessary security credentials for access to a federal facility.
- Must successfully complete a background investigation prior to start.
- Valid driver's license and reliable transportation required; regular in-state travel is expected.
- Proficiency in Microsoft Office Suite and electronic health record (EHR) systems required.
