    Program Description:


    The WISe program provides an integrated and flexible array of strengths-based services and intensive supports to families with children who have serious mental health and other needs.

    All services are delivered with the following core values in mind:


    safety of the child, family and community, flexible and responsive, family driven; normative, family oriented, strengths based, comprehensive and collaborative, unconditional, individualized and culturally relevant.


    Position Description:


    Incumbent is in a professional position providing intensive, community-based therapeutic services to children, youth and families with serious mental health needs.


    Incumbent will utilize the Wraparound process to increase safety, stabilize placement and support individuals/families in meeting their identified needs by building a team of natural and formal supports.

    Incumbent will participate as a clinical team member, learning and practicing Wraparound principles and processes. Incumbent will participate in crisis management and the on-call rotation.

    Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.


    Responsibilities:
    Universal Job Skills

    Communicate role, responsibility and agency philosophy in order to assure mutual respect, confidence and trust with the child and family and other stakeholders.

    Provide non-judgmental, unconditional support to the child and family.
    Assess for immediate safety and stabilization needs.
    Customize helping approaches to fit the familys uniqueness, personality, culture and interest.
    Report relevant information to the right people at the right time.
    Maintain a focus on strengths, needs and creative solutions.
    Utilize the familys expertise in problem solving and solution seeking.
    Work interdependently with others toward common goals.
    Respond to familys and childrens needs in a timely fashion.
    Provide and/or coordinate client transportation as needed.
    Complete required documentation within established time-frames.
    Instill hope for the future by communicating and behaving with confidence and reassurance.


    Welcome, Engagement and Assessment:
    Meet and engage the family communicating compassion, support, respect and enthusiasm for them and your role as a helper.
    Listen actively to the family and, with them: identify strengths, needs, preferences and goals.
    Provide and/or arrange necessary crisis response and stabilization services, completing and communicating the safety/crisis plan with family and others.

    Contact potential team members, including other system partners, to gather their perspective on the familys strengths and needs and solicit participation and commitment to a team process.

    Prepare the family for next steps in the intervention (e.g., team development, planning process, natural supports, etc.).
    Introduce and complete paperwork appropriate to the situation (including assessment instruments, etc.).
    Analyze and align the familys position and agency mission using methods of respect, communication and conflict resolution.


    Individualized Service Planning:

    Facilitate the child and family team to:
    Identify and prioritize goals and needs.
    Communicate and translate information in a way that everyone can hear especially the familys position.
    Brainstorm and negotiate strategies that build on child and family strengths.
    Summarize and write up the care plan.
    Consult with clinical supervisor, mental health specialists and others as needed and incorporate recommendations.


    Implementing Services:

    Provide direct helping interventions that:
    Respond to crises in a prompt, effective and collaborative manner.
    Provide and/or facilitate the provision of a range of therapeutic responses that support the overall care plan.
    Utilize parent/family expertise in problem solving around specific needs.
    Modify and adjust individual intervention techniques for each situation without changing the direction of the plan.

    Partner with the family in order to assist them in utilizing the resources offered by formal and informal sources within their community.

    Seek assistance and report incidents to the right people at the right time.
    Facilitate others, including a team, to implement the care plan from beginning to successful outcome:
    Communicate plan, strategies and role to team members, including involved CCS staff.
    Consult with, recommend and provide training to team members around specific interventions.
    Maintain a focus on strengths, needs and creative solutions and inspire others to follow this format in problem solving.
    Reevaluate, modify, and redesign care plan based on new information.

    Transitioning Services to Natural and Other Community Supports:
    Inspire confidence in the child, family and other team members about their strengths and ability to transition successfully.
    Arrange and negotiate a process for ongoing formal and informal services and supports.
    Create and practice a plan for transition and/or aftercare supports and response and post- Wraparound involvement.

    Access to and protection of PHI:
    Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).

    Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).


    Other work related duties as assigned:
    Share in on-call rotation for 24-hour response to WISe families.


    Qualifications:
    BA degree.
    1 year direct experience with youth and/or families in a structured professional setting.
    Ability to work in home and community settings.
    Must have reliable transportation, valid drivers license and automobile insurance.
    Must provide proof of acceptable driving record (Drivers Abstract-Full Record).
    Ability and willingness to obtain WA State Agency Affiliate Counselor Certification.
    Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
    Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services.
    Support and contribute to a creative, collaborative and respectful environment that promotes teamwork.
    Criminal History Background Checks are required prior to employment.


    PREFERRED:
    BA degree in social services or related field
    Two years or more experience in childrens mental health.
    Basic knowledge of wraparound principles and practice.
    CPR/First Aid (Infant/Child/Adult).
